What to Bring & How to Prepare for Your Visit

A little preparation goes a long way. Please bring a photo ID, your insurance card, and a list of any medications you take. If you are new to our office, plan to arrive about 15 minutes early so you have time to settle in and finish any last paperwork. Returning patients should also bring updated insurance information if anything has changed since your last visit.

Forms made simple

Completing Your Dental Patient Forms

Getting your dental patient forms done ahead of time helps your visit start on time and run smoothly.

New Patient Forms

New patients are asked to complete a health history and general information form before their first appointment. Completing these ahead of time means less waiting once you arrive and gives Dr. Isom a clear picture of your dental and medical background before your exam begins.

Updating Forms for Returning Patients

Returning patients only need to update forms when something changes, like a new medication, a new insurance plan, or a change in your health history. Our front desk team can help you update this information by phone or when you check in.
